The show opens with Dr. Drew apologizing to Adam for some of the fogginess that he has been experiencing that has led to his forgetting to return some of Adam's calls. This leads to a larger metaphor of people's mental capacity and ability to improve or degrade that capacity in a variety of ways. They then turn to the phones and speak to a variety of callers on topics from career change to conceiving a baby and more.
